Surrender Application

This is an application to surrender and does not constitute an agreement by Wildhearts Rescue to take in your dog.
We will let you know if we are able to accept your dog upon receipt of a completed form and the required documentation.

Please email us:

- A copy of your dog’s most recent vaccination record and any documents of past medical care.

- 3-5 photos of your dog (timeline photos are great for future adopters to see the dog at all ages)

In the subject line please state “Surrender Info: ‘Dog’s Name’”
